Retail Opportunities You may become a New Hampshire Lottery® retailer and here is how. Please print, complete and sign the forms listed in step 3 and return to the NH Lottery, 14 Integra Drive, Concord, NH 03301 for processing. Please be advised that a part of our processing includes an investigation of your personal credit history. Of course, all information is kept strictly confidential. Please make sure to include the following items in the packet you return to us: 1. Employer ID Number or Social Security Number for tax purposes. 2. DEPOSIT SLIP OR VOIDED CHECK (very important!) for electronic fund withdrawal form. 3. All four forms below completely filled out and signed by the appropriate persons. Click on the name of the form and print. You must have Adobe Reader installed on your computer to open and print these forms. Download Adobe Reader.
Once the paperwork arrives at our office, we will begin processing it for you. Part of this processing will include a physical survey of your location, to ensure that our retailers are in compliance with The Americans with Disabilities Act (ADA), that became law in 1990. This will be done by one of our sales staff. If your location does not meet ADA requirements we will notify you in writing and you will have 30 days to comply. Prior to the terminal becoming active you must attend a training class at the NH Lottery™ office in Concord, NH. You will be notified of the upcoming classes. Please allow 3 to 4 weeks for completion of all the steps necessary to process your application. IMPORTANT: If this is a new Lottery location please be advised that it is our responsibility to order and maintain the telephone line that will connect to your terminal. Once your application has been approved we will request Verizon to install the necessary line. This line must be completed by Verizon and end within 2 ft of the proposed terminal location within your business.
